
What is a COC?
“The Continuum of Care (CoC) Program … is designed to promote a community-wide commitment to the goal of ending homelessness; to provide funding for efforts by nonprofit providers, states, Indian Tribes or tribally designated housing entities… and local governments to quickly rehouse homeless individuals, families, persons fleeing domestic violence, dating violence, sexual assault, and stalking, and youth while minimizing the trauma and dislocation caused by homelessness; to promote access to and effective utilization of mainstream programs by homeless individuals and families, and to optimize self-sufficiency among those experiencing homelessness.”
MCHH Mission:
We are a consortium of housing, emergency shelter and supportive services providers that work alongside community based organizations and stakeholders to address homelessness in our community.
The MCHH operates under the umbrella of the City of Milwaukee which serves as Milwaukee’s Lead Agency.

“The Collaborative Applicant is the entity the CoC designates to submit the annual CoC Program Competition application and apply for planning funds on its behalf. The entity must be an eligible applicant, which includes a private nonprofit organization, State or local government, or an instrumentality of State or local government. HUD has not set a process CoCs must follow to select a Collaborative Applicant. Rather, the CoC must determine a process to make this decision and formalize it in its governance charter. A CoC does have the option to change its Collaborative Applicant at any time so long as the CoC follows its written process.”

“HMIS [Homelessness Management Information System] is a local information technology system used to collect client-level data and data on the provision of housing and services to individuals and families at risk of and experiencing homelessness. Each CoC is responsible for selecting an HMIS software solution that complies with HUD's data collection, management, and reporting standards.”

“Coordinated Entry (CE) is the starting point for those experiencing homelessness in Milwaukee County to access emergency shelter and housing resources. We utilize a standardized process that ensures equitable access to available programs serving single adults, families, and individuals fleeing domestic violence, community violence, or trafficking. Although our team does not deliver direct services, we help individuals navigate the system that provides case management, financial assistance, eviction prevention, shelter placement, housing and other supportive services to eligible clients.”
